Presently, (name(s) (is/are)) listed as my (Beneficiary/Beneficiaries).  I would like (him/her/them) to be replaced with (name(s) of new beneficiary/beneficiaries).  All of the other information, such as amount and disbursement instructions, should remain unchanged.
